[EHPweb] EHP Wiki Transition Complete

Okay all,

The new EHP Wiki has been migrated over from the doku site 
(https://ehpmaster.wr.usgs.gov/admin/wiki/doku.php)
to the new site (http://ehptools.cr.usgs.gov/trac/wiki). 

Any future changes to the wiki, please use the new site.  For the most 
part, the migration went very smoothly with
the exception of a couple of the pages that implemented forms.
