[Shake-dev] The format of the *_fault.txt ??

Hello,

Your fault definition is incomplete. Each point should include latitude, longitude, and depth. In addition, each of your quadrilaterals should include five points, with the last point being the same as the first.

See section 3.5.4.4 “A Note about Finite Faults” in the Software Guide.
