[Shake-dev] more on offline plotting

Doesn't answer Steve's questions but I had a need to quickly add stations
offline not included in the online generated maps.  To ease the pain of
the signal processing I wrote a crude sac2gm_xml that uses Pete's ew
libs to help generate the xml for the usual groundmotion types.  Has
hard-wires to sac and s.c.n and doesn't read a loc or arrival table
(instead reports the peak of the whole snippet rather than a search
around S).  Others are welcome to it if they wish.
