Julio,
Your example (a fault with sole characteristic M7 earthquakes with recurrence interval = 250 years that lead to PGA ~ 0.4 g on average in a site) demonstrates that how PSHA can mislead to a zero PGA (no seismic design consideration) if a 250-year return period is considered. Your example also demonstrates that how PSHA can "create" ground motions (events) from a single earthquake.
